Jordan Valley Community Health to launch southwest Missouris first Program of All-Inclusive Care for the Elderly (PACE) [Video]

SPRINGFIELD, Mo. (KY3) – Jordan Valley Community Health Center (JVCHC) is about to launch a Program of All-Inclusive Care for the Elderly (PACE) in the Springfield area. PACE care is designed to be a one-stop-shop senior citizen health provider that includes medical and personal care, rehabilitation, social interaction, medications, transportation, and more; Jordan Valley Senior Care is the first PACE model to launch in southwest Missouri and the third in the state of Missouri. The program is launching the assessment and enrollment process and should begin caring for participants in July.

Administered by Missouri HealthNet and Medicare, the program’s primary goal is to help older adults live safely in their homes instead of moving to nursing facilities.
