Exploring Dementia Care and Support Services – Breakout Session – 2024 AAAA Caregiver Conference

Heidi Haley-Franklin is the Vice President, Programs at the MN ND chapter of the Alzheimer’s Association in Edina, MN. Heidi has over 20 years of experience working with individuals and families in private practice, group homes, long-term and home health care settings. In her current position, she oversees all of the Association’s programs and services, and provides clinical supervision and ongoing education to those who directly work with individuals impacted by Alzheimer’s disease and related dementias. Heidi holds a master’s degree in social work from the University of St. Thomas in St. Paul, MN, a BA from the University of MN, Morris, and is a Licensed Independent Clinical Social Worker.

Mimi Stender is the founder of Duluth Aging Support, a nonprofit organization that works to enhance the lives of older adults and caregivers in the Northland through resources, outreach, awareness, advocacy, and collaborative partnerships. Mimi is passionate about bringing together organizations and individuals with a common mission to improve community support and the quality of life for community members, especially under-resourced community members. Mimi co-leads the Duluth Age-Friendly Initiative and serves on the Governor’s Council for an Age-Friendly Minnesota. As a volunteer for the Alzheimer’s Association, Mimi co-facilitates two caregiver support groups in Duluth. In 2023, she received her Graduate Certificate in Interprofessional Healthy Aging from Arizona State University’s Edson College of Nursing and Health Innovation. She is a long-time Duluth, MN resident and graduate of Colorado College, Colorado Springs, CO.

This presentation will: Explain why it’s important to plan for dementia care; Describe the kinds of respite care that may be available, including both informal home and community care; Compare different kinds of long-term and residential care; Describe end-of-life care options; Know what is important when moving a person living with dementia from one setting to another; Discuss Alzheimer’s Associations resources including education and support groups.
