Have you thought about who will be your voice if you’re ever in need?

For National Advance Care Planning Week (18-24 March), our CWHHS Palliative Care team want to let you know how you can have those sometimes difficult, but valuable conversations ahead of time.

The Enduring Power of Attorney form will help you nominate someone close to you who you trust with making important health and financial decisions on your behalf if you’re ever not in a position to do so.
